Upon arrival and check-in, new patients will be asked to fill out a
comprehensive personal and family history questionnaire as well as
read our privacy policy statement. We recommend that you arrive at
least 20 minutes prior to your appointment time so that you can read
these forms carefully and be seen on time.
Copies of our forms are
available on our website. You can download them and fill them out at
home, before coming to the office, in order to decrease waiting time.
All new patients receive dilated examinations that can take a full
hour to complete. Please plan accordingly. |

Our office participates with many major insurance carriers. Patients
should bring their insurance card and any referrals or authorizations
as required by the plan, to your appointment. If these items are
missing, we may not be able to see you at your scheduled time. All
payments are due at the time of service; this includes co-payments and
charges applied toward unmet deductibles. Personal checks as well as
Mastercard and Visa credit cards are accepted.
Claims are submitted directly to the (participating) plans we
participate with. If we do not participate with your insurance plan,
payment in full will be expected at the time of service. If you choose
to submit your own claim, any payments we receive will be refunded to
you. Payment for balances deemed patient responsibility is due upon
receipt of the statement. If a payment plan is needed, our office can
arrange a time payment plan. Balances unpaid for more than 90 days
will be referred to a collection agency. We, however, would always
prefer to work with our patients.
If you do call the business office, depending on the call volume, you
may be prompted to leave a message. Responding to your inquiries is
very important to me. In most cases, you will receive a return call
within 24 hours. However, depending on the issue, additional time to
research your account may be necessary. You may also
